Everbridge App and Symptom Checker


The Daily Symptom Checker is part of our health screening and tracking process. Completing this questionnaire is a crucial part of keeping our campus safe.

You are required to complete the Daily Symptom Checker on the day of your approved visit before entering Hostos or any other CUNY facility. The Daily Symptom Checker can be found in the Everbridge App (iOS/Android) on your mobile device, and can be completed before you arrive. (If you do not have access to a smartphone that can run the Everbridge App, you can sign up for daily reminder emails.)

After a brief one-time setup of Everbridge, the Daily Symptom Checker takes less than two minutes to complete. The questions asked are an important reminder to consider whether you have symptoms of—or have been exposed to someone with—COVID-19 before you step foot on campus.

This questionnaire is important, and it deserves your undivided attention!

When you complete the Daily Symptom Checker:
  • Find a place with a stable internet connection before you start. Answering on the subway or bus, or in a car can cause problems when using the Everbridge app.
  • Make sure you read each question fully before selecting your answer and hitting the “Next” button. For safety reasons, there is no “Back” button or other way to return to a previous response, even to correct an error.

When you’ve answered all the questions in the Daily Symptom Checker, you’ll see one of two time-stamped tokens.
  • If see a green check mark ✔ token, you’re approved to visit. You'll need to show this token and your Hostos ID to the health check-in station staff when you arrive.Image of green "Approved" token from Everbridge Daily Symptom Checker

Note: some users have reported that they need to connect their mobile device to wi-fi to view their token after completing the Symptom Checker. If you do not receive your token after a few minutes, please connect to a wireless network and try refreshing the Everbridge app.

 

Using the Everbridge App

Instructions for Students, Faculty, & Staff

One-Time Setup
  • Step A: Download and install the Everbridge app from your mobile device app store (iOS/Android).
  • Step B: Click the “Find an organization or subscription” button on the Everbridge splash page.
  • Step C: Search for “CUNY” and select the “CUNY – Health Screening” option. (Do not choose “NY Alert – CUNY.”)
  • Step D: Clicking “CUNY – Health Screening” will take you to the CUNY Web Applications Login page. Use your CUNY Login credentials. (Your username is your CUNYfirst username, with "login.cuny.edu" at the end (e.g. Firstname.Lastname##@login.cuny.edu). Your password is the same one you use to log in to CUNYfirst.)
  • Step E: Set your notification preferences and click the “Done” button.

Using the Daily Symptom Checker through the Everbridge App
  1. Click the shield icon at the bottom right corner of the Everbridge app. (If you do not see the Shield icon, restart the app.)
  2. Click the Symptom Checker icon.
  3. Carefully complete the questionnaire. Make sure you read each question fully before selecting your answer; for safety reasons, there is no way to return to a previous response, even to correct an error.
  4. Once you've clicked “Submit,” you can close the Symptom Checker.
  5. After a few minutes, you’ll receive a notification that the result of your screening is in your “Feed” in the Everbridge app.
  6. Your token is now available in your Everbridge Feed. If you’ve received an approval confirmation, you can enter the campus. Open the app when you arrive, and click on this item in your feed. Show the token and your Hostos ID to the health check-in station staff.

App Instructions for Visitors

Prior to Your Visit
  1. Download and install the Everbridge app from your mobile device app store (iOS/Android).
  2. Open the Everbridge app once to ensure it works and allow notifications.
During Your Visit
  1. When you arrive at the Heath Check-In Station, provide the screener with your first and last name, and email address.
  2. You will receive an email from noreply@everbridge.net with a link to Everbridge registration. Click on the "Click here to register" link. 
  3. Click the "OK" button to confirm that you want to launce the Everbridge app
  4. Click the shield icon at the bottom right corner of the Everbridge app. (If you do not see the Shield icon, restart the app.)
  5. Click the Symptom Checker icon.
  6. Complete the questionnaire by answering all five questions, and click “Submit.”
  7. Close the Symptom Checker. After a few minutes, you’ll receive a notification that the result of your screening is in your “Feed” in the Everbridge app.
  8. Your token is now available in your Everbridge Feed. Open the app and show this token to the health check-in station staff.

Using the Everbridge Member Portal & Daily Emails

Email Instructions for Students, Faculty, & Staff

One-Time Setup
  1. Go to CUNY's Everbridge Member Portal at https://member.everbridge.net/index/509627934441532.
  2. Click the "Sign in with SSO credentials" button. Sign in with your CUNY Login credentials. (Your username is your CUNYfirst username, with "login.cuny.edu" at the end (e.g. Firstname.Lastname##@login.cuny.edu). Your password is the same one you use to log in to CUNYfirst.)
  3. Once you've logged in. find and click the "Edit" link at the top right of the "My Subscriptions" section. Choose your campus to receive the daily health screening emails. Click Save.
Using the Daily Symptom Checker through the Everbridge App
  1. Every day you will receive an email reminder from CUNY Wellness Survey with the Daily Symptom Checker questionnaire. The email will be active for the next 24 hours only.
  2. Carefully read over the questions. Submit one "yes" or "no" answer for all 5 questions by clicking the link at the bottom of the email message. (Answer "yes" if one or more of the questions applies.)
  3. Selecting your answer will open a new webpage in your browser. You will see a message in your browser that confirms you have successfully completed the health screening questionnaire.
  4. ​Within several minutes, you will receive an email from CUNY Wellness Survey with your token. If you receive a green approval token, you can enter the campus. Show the token and your Hostos ID to the health check-in station staff.

 

Email Instructions for Visitors

Prior to your Visit
  1. When you email CampusVisit@hostos.cuny.edu to request approval from Public Safety to visit the campus, inform Public Safety that you need to complete the Everbridge Daily Symptom Checker through email.
  2. You will receive an email with the subject line “Member Portal Registration Invitation from CUNY.” Click the registration link inside the email.
  3. Complete fields for username/password on the Sign Up page. Click Create Your Account to continue.
  4. Enter your contact information on the My Profile page. Click Save & Continue.
  5. Select your language preference, guest contact type and guest campus on the My Information page.
  6. Review your information and click the Finish button.
During your visit
  1. You will receive an email reminder from CUNY Wellness Survey with the Daily Symptom Checker questionnaire. The email will be active for the next 24 hours only.
